Sanguine's Tear
A crimson shard that remembers every wound it was born from.
Sanguine's Tear is a unique artifact in Grim Dawn, the dark-fantasy action RPG developed by Crate Entertainment and published by Eleventh Game. It occupies the tier of named, single-instance gear that players seek through exploration, boss encounters, and world events across the continent of Kaitain.
As a tear-shaped relic, it fits within Grim Dawn's broader tradition of blood- and curse-themed unique items that reward players who invest in visceral, high-risk build archetypes. It is not a common drop but a singular piece of equipment with fixed affixes that define a particular playstyle niche.

Lore & Background
In the old histories of Kaitain, before the Sundering fractured the land and the Warden Order was raised to hold the breaches in check, there existed a lineage of blood-pact sorcerers who drew power from their own vitality. Sanguine's Tear is said to be the crystallized last drop of one such sorcerer's essence, shed at the moment of a failed ritual that tore a wound in the veil between the mortal plane and the Abyssal depths. The tear did not shatter; it hardened, retaining a faint, pulsing warmth that no alchemist in the Free Cities has been able to fully explain.
Over the centuries the relic surfaced in the hands of various factions—Wardens who studied it in their vaults, cultists of the Abyss who sought to reconstitute the sorcerer's will, and merchants in the Shattered Coast who traded it as a curiosity before its true potential was understood. Its presence on the battlefield is always accompanied by a faint metallic scent and a low, rhythmic thrumming, as though a heart still beats within the glass.
Within Grim Dawn's item system, the Tear functions as a fixed-affix unique piece. It does not randomize; every instance carries the same set of modifiers, making it a targeted goal for players building around specific damage types or resource-management loops. Its design philosophy mirrors other named artifacts in the game: a clear mechanical identity that rewards commitment to a particular build philosophy rather than offering broad, jack-of-all-trades utility.
In Their Own Story
The campfire crackled in the Ashlands' perpetual amber haze. Sera set the Tear on the flat stone between them, and the light it gave was not warm—it was the red of a vein just opened, steady and unhurried. Her companion, a Warden surveyor with a scarred jaw, leaned closer and whispered, "You're sure this is the one the codex described? The one that hums in the key of a dying pulse?"
Sera said nothing. She turned the Tear over in her gloved fingers. It was lighter than it looked, and the surface was not smooth but faintly textured, like the inside of a throat. When she held it up to the firelight, the glow deepened, and for a moment the shadows around the fire bent inward, as if listening.
"It's not a weapon," she said finally. "It's a debt. And I'm about to collect."
Reader's Guide
Base identity: Sanguine's Tear is a fixed-affix unique artifact in Grim Dawn. Because it is unique, it carries a predetermined set of modifiers rather than random rolls, and it occupies a single equipment slot (accessory/amulet tier). It cannot be re-rolled, socketed with additional gems beyond its inherent design, or altered by the Blacksmith beyond standard durability repair.
Affix profile (general): The Tear's modifiers lean toward blood- or life-cost-based damage scaling, critical-strike enhancement tied to low-health thresholds, and a feedback loop that converts a portion of damage taken into temporary offensive buffs. This places it squarely in the "risk-reward" build category common to Grim Dawn's Warden and Mercenary archetypes.
Drop context: As a unique artifact, it is not a guaranteed vendor item. It can appear as a world drop in higher-difficulty content, from elite or named enemies in the mid-to-late game zones, and occasionally from specific world-event rewards. It is not tied to a single mandatory quest chain, meaning players may encounter it across multiple playthroughs and difficulty tiers.
Build role: The Tear excels in builds that intentionally dip into low-health windows—vampire-style sustain, bleed-damage stacks, or curse-amplification loops. It synergizes with gear that increases life-cost effectiveness and with skills that apply or amplify bleeding, making it a centerpiece for aggressive, close-range Warden or Mercenary configurations rather than a ranged utility pick.
